Skurczowe ciśnienie tętnicze (SBP) wykazuje wyraźną, liniową, wprost proporcjonalną zależność od wieku chorego. Zdecydowanie największym problemem terapeutycznym z nim związanym są jego podwyższone wartości u pacjentów powyżej 50. roku życia, bardzo często przybierające postać izolowanego nadciśnienia skurczowego (ISH).Trzeba zaznaczyć, na podstawie dużych projektów epidemiologicznych (NHANES III, WISHE, WHO MONICA), że problem ten dotyczy blisko 65% ogólnej populacji chorych na nadciśnienie tętnicze niedostatecznie leczonych i aż 80% pacjentów po 50. roku życia. W patogenezie wzrostu składowej skurczowej nadciśnienia wraz z wiekiem podstawowe znaczenie przypisuje się narastającej sztywności dużych naczyń tętniczych (aorty i rozgałęzień), upośledzonej wazodylatacji w odpowiedzi na stymulację receptorów beta-adrenergicznych, przy zachowanej reakcji na stymulację receptorów alfa uszkodzeniu śródbłonka i związanym z wiekiem zmianom w nerkach. Wyniki wieloletnich obserwacji epidemiologicznych, m.in. z badania Framingham czy projektu Monica, wykazały wprost proporcjonalną zależność między ciśnieniem skurczowym a powikłaniami sercowo-naczyniowymi i niewydolnością nerek już od wartości powyżej 115 mmHg (...)

Citalopram to lek z grupy selektywnych inhibitorów wychwytu zwrotnego serotoniny. Został zarejestrowany do leczenia depresji, jednakże szereg badań wskazuje na możliwości jego zastosowania również w leczeniu niektórych zaburzeń lękowych oraz zaburzenia obsesyjno-kompulsywnego. Warto pamiętać, że w pewnych sytuacjach klinicznych zastosowanie citalopramu może okazać się optymalnym postępowaniem. W artykule przedstawiono historię trzech pacjentów, u których zastosowanie citalopramu okazało się skutecznym podejściem do leczenia depresji, które pozwoliło na uniknięcie potencjalnych działań ubocznych związanych ze stosowaniem leków przeciwdepresyjnych. Opisy przypadków przedstawiono w szerszym kontekście istniejącego stanu wiedzy dotyczącego farmakoterapii depresji.
