Postępowanie międzyinstancyjne w postępowaniu sądowoadministracyjnym obejmuje swym zakresem zespół czynności podejmowanych w nich tak przez skarżących jak i sądy pierwszej instancji. W szczególności wyposażenie tych ostatnich w kompetencje samokontrole, zarówno w postępowaniu kasacyjnym jak i zażaleniowym, oznaczało dla ustawodawcy konieczność zbalansowania potrzeby ochrony interesu jednostki w szybszym zakończeniu postępowania a z drugiej strony ochronę obiektywnego porządku prawnego, wyrażającego się w tym przypadku w nienaruszeniu konstytucyjnej zasady dwuinstancyjności postępowania. Monografia opiera się na założeniu, że postępowanie międzyinstancyjne w postępowaniu sądowoadministracyjnym, stanowiąc pierwszy etap postępowania odwoławczego, ma miejsce w przypadku modelu środków odwoławczych posiadających przymiot dewolutywności. Aby zmaksymalizować szanse na skuteczne leczenie, kluczowe jest dostosowanie podejścia terapeutycznego do celów i oczekiwań pacjenta oraz ocena potencjalnych skutków ubocznych, kosztów i spodziewanych korzyści. Znajomość problematyki bólu przewlekłego, w tym bólu przebijającego (epizodycznego), jest niezbędna wśród lekarzy, pielęgniarek, fizjoterapeutów, psychologów i innych osób wchodzących w skład zespołów interdyscyplinarnych opiekujących się chorymi na nowotwory. W monografii omówiono ogólne zasady leczenia bólu u pacjentów z nowotworami, charakterystykę bólu przebijającego z uwzględnieniem oceny klinicznej, jego cechy, epidemiologię, patomechanizm, podział oraz różnicowanie z bólem podstawowym. Wirus SARS-CoV-2 wywołujący chorobę COVID-19 (coronavirus disease 2019) stał się przyczyną pandemii, która spowodowała niespotykane dotąd globalne następstwa zdrowotne, społeczne i ekonomiczne. Na całym świecie zachorowało ponad 200 mln osób, w Polsce ok. 3 mln. Obserwacje kliniczne wskazują, że wielu chorych na COVID-19 po ostrym okresie choroby, gdy w ich organizmie nie wykrywa się już RNA wirusa, nadal zgłasza przewlekłe dolegliwości/objawy wynikające z zaburzenia funkcji różnych narządów i układów. Zjawisko utrzymywania się dolegliwości/ objawów po upływie 4 tygodni od początku infekcji SARS-CoV-2 określono mianem zespołu post-COVID. Dostępne doniesienia na temat następstw zdrowotnych COVID-19 przemawiają za tym, że zespół post-COVID jest dość częstym zjawiskiem. Większość obserwacji wskazuje, że cięższy przebieg infekcji SARS-CoV-2 (definiowany np. jako potrzeba hospitalizacji na oddziale intensywnej terapii czy potrzeba biernej i/lub czynnej wentylacji) predysponuje do wystąpienia zespołu post-COVID, w tym również zwiększa ryzyko wystąpienia objawów ze strony układu nerwowego. Symptomy te mogą występować w przebiegu fazy ostrej, podostrej i przewlekłej COVID-19, co zasadniczo określa się jako zespόł neuro-COVID. Zajęty może zostać zarówno ośrodkowy, jak i obwodowy układ nerwowy. W pierwszym przypadku objawia się to najczęściej incydentami naczyniowymi, chorobami zapalnymi mózgu i zaburzeniami funkcji poznawczych pod postacią tzw. mgły COVID-owej, w drugim - najczęściej zaburzeniami węchu i smaku oraz różnopostaciową neuropatią i miopatią. Infekcja SARS-CoV-2 może również prowadzić do zaostrzenia przewlekłych chorób neurologicznych, a także wywierać wpływ na dotychczasowe leczenie. Wszystkie te powikłania wymagają interwencji terapeutycznych. Tak jak pandemia zaskoczyła świat, tak samo wciąż są poszukiwane i na bieżąco praktykowane nowe sposoby leczenia powikłań. W przypadku powikłań neurologicznych - zwłaszcza w celu zapobieżenia incydentom naczyniowo-mózgowym oraz poprawienia funkcjonowania poznawczego w przebiegu mgły COVID-owej - celowe i skuteczne wydaje się zastosowanie nicergoliny.

Przybliżamy temat diagnostyki zaburzeń hormonalnych i wspominamy, jakie trudności można napotkać przy wprowadzaniu hormonalnej terapii menopauzy. Nie zawsze wprowadzenie leczenia hormonalnego jest możliwe lub wskazane. Istnieje wiele sytuacji, w których nie decydujemy się na ten krok; rezygnacja z hormonalnej terapii menopauzy nie oznacza jednak, że ignorujemy objawy i trudności doświadczane przez pacjentkę. Okres okołomenopauzalny jest czasem, gdy powinniśmy zachować szczególną czujność odnośnie do możliwości wystąpienia u pacjentek objawów depresyjnych. W tym kontekście podkreślamy rolę lekarza rodzinnego we wczesnym wykrywaniu i leczeniu depresji. Towarzyszące menopauzie objawy ze sfery psychicznej mogą w wielu przypadkach być skutecznie leczone w warunkach praktyki lekarza podstawowej opieki zdrowotnej. Czasem wdrożenie leczenia farmakologicznego może w szybki sposób poprawić samopoczucie i komfort funkcjonowania pacjentek.
